OUR HCS GROUP HOMES
Our residential living options are designed to create a traditional home-like atmosphere for our clients. The assistance provided encompasses essential aspects of personal care, independent living tasks, health maintenance, and engagement in community activities. This holistic approach aims to support individuals in developing, retaining, and enhancing their community living skills.
OUR SERVICES
Residential Services
The HCS program provides residential services to persons with IDD in the home that helps support the person maintain themselves in the community and have opportunities to participate as a citizen to the maximum extent possible.

Nursing Services
Nursing is provided to people who require treatment and monitoring of health care procedures .
Employment Assistance
Employment assistance helps people locate paid jobs, and includes helping them: • identify employment preferences, job skills, and work requirements and conditions; and • identify prospective employers who offer appropriate employment.
Adaptive Aids
An adaptive aid is an item or service that enables a person to retain or increase their ability to perform activities of daily living or function in their environment. The repair and maintenance of a billable item that is not covered by warranty is also billable for the adaptive aids service.

Dental Treatment
Dental services up to $2000 per year are provided for persons who are over the age of 21.
Supported Employment
Supported employment is provided to a person who has paid employment to help him or her sustain that employment. It includes individualized support services, supervision and training.
Minor Home Modifications
A minor home modification is a physical adaptation to a person’s residence that is necessary to address the person’s specific living environment needs and that enables the person to function with vaster independence in the person’s residence. The repair and maintenance of a billable adaptation not covered by warranty is also billable for the minor home modifications service.
Individual Skills and Socialization

Individual Skills and Socialization is assistance with getting, keeping or improving self-help, socialization and
adaptive skills necessary to live successfully in the community and to participate in home and
community life. Individual Skills and Socialization is normally provided regularly in a group setting (not in the person’s residence) and includes personal assistance for those who cannot manage their personal care needs during day habilitation, and assistance with medications and performing tasks delegated by a registered nurse.
PROFESSIONAL SERVICES
Behavioral Supports
Behavioral supports are specialized interventions to help people increase adaptive behaviors and to replace or modify maladaptive behaviors that prevent or interfere with their inclusion in home and family life or community life. Supports include assessing and analyzing assessment to develop an individualized behavior support plan consistent with the outcomes identified in the person-directed plan, as well as training and consulting with family members or other providers.
Specialized Therapies
Specialized therapies include assessment and treatment by licensed or certified professionals for social work services, counseling services, occupational therapy, physical therapy, speech and language therapy, audiology services, dietary services and behavioral health services other than those provided by a local mental health authority and training and consulting with family members or other providers.
